		Freedom of Information Act The Freedom of Information Act form can be brought to the Village Hall, mailed or faxed.
		Water & Sewer Bill Direct Debit Form The Water & Sewer Bill Direct Debit form can be brought to the Village Hall, mailed or faxed. Please be sure to include a voided check or a copy of a check from the account you wish to have the bill debited out of.
		Motor Vehicle License Fee Reduction/Exemption Application The Motor Vehicle License Fee Reduction/Exemption Application needs to be witnessed by a Village employee. Please bring it to the Village Hall during normal business hours.
		Block Party Application In order to grant permission to close a street, the Village must obtain some important information to ensure the safety of those who reside on the block. This form only needs to be completed and returned to the Lindenhurst Police Department if you are interested in having your road closed.
		Homeowner Comprehensive Guide to Parkway Tree Planting Selecting and planting parkway trees can be a very daunting experience. To simplify this process, the Village of Lindenhurst has established this simple but comprehensive guide to planting. This form includes a parkway tree planting guide, Village approved parkway tree listing, and a public property tree work permit application. If you have any questions regarding this process please contact the Village Arborist at 847-356-8252.
		Local Building Alarm System Permit The Application and Agreement for Local Building Alarm System Permit form can be brought to the Village Hall or mailed. You need to include $50 to cover the cost of the permit. If you have any changes to the emergency contact information, please contact the Lindenhurst Police Department (847-356-5400) to ensure proper notification of alarms. False alarms are billed on a calendar basis. January 1st of each year you are allowed 3 false alarms at no charge. The next 3 (4-6 yearly) are billed at $25 each, the next 3 (7-9 yearly) are billed at $50 each, and each false alarm thereafter is billed at $100 each.
